01

THE SITUATION

All revenue came from one payer.

The agency served about 40 clients with a similar number of caregivers, roughly 600 hours of care a week. All of that revenue came through one payer, the state Medicaid program, so a single rate cut or policy change could hit most of its income, and that payer's rate capped what each hour of care could earn.

Growth had two other limits. Every new client needed a new caregiver, and the owners handled scheduling, quality checks, hiring and client calls themselves.

02

WHAT WE FOUND

Two channels the agency could not reach on its own.

We mapped every referral and payment channel open to a personal support agency in Maine: eight in all. Two stood out. One was a referral network for long-term care insurance policyholders that the owner had not heard of. The other was a government network for veterans' care.

Both gave the agency access to clients it could not reach before. We recommended starting there.

Two new referral partners

A long-term care insurance network reaching three national insurers, and the VA's community care network, opened from the first call through approval.

Hiring

A redesigned recruiting, screening and selection process, so hiring could keep pace with new clients.

Referral to paid claim

A claims guide, an eight-step client workflow, billing separated by payer and outreach to regional veterans' clinics.

Clear priorities

A new org chart, clear owners for finance and HR, and an office manager role for daily administration.

Compliance and systems

47 forms audited and finalized, a caregiver orientation, core policies and a new management platform.

Brand and digital presence

A rebuilt brand and website that produced 22 leads from February to September 2026, with no paid advertising.

03

WHAT WE BUILT

The channels, and the operation to serve them.

“They tackled our biggest operational constraint, our one-to-one ratio of new clients to new hires, by redesigning our recruiting strategy and improving our screening and selection process. … What made the biggest difference was having a team to think strategically alongside me. I spend every day working in the business; they helped me work on the business.”

Wendy, CEO of PHCM

OUR RESULTS

New revenue that continued after the handoff.

The new channels brought in revenue quickly. The operation behind them freed the owners to run the business, and everything we built stayed with the agency when we left.

Ready for accreditation

The agency was prepared for accreditation review.

Every asset in their name

Every account, template and document we built belongs to the agency.

Growth after the handoff

The engagement ended in August 2026, and the agency has kept adding clients since.

Revenue is the current billing of the 10 new clients, annualized as of October 2026, with nothing deducted. It includes one state-funded client who came through the new website. Gross profit is that revenue minus the cost of providing caregivers. Growth from the agency's existing referral sources is excluded.

Results reflect an individual engagement and are not typical or guaranteed.
